Medication Administration Forms 2023-2024
The Department of Education has released Medication Administration forms in English. Forms translated into other languages will be available soon. If your child takes medication daily, you must first submit a completed form. Your child’s physician completes the bulk of the form and a parent/guardian completes part of the form. The school nurse reviews the form and then contacts the parent/guardian to bring the medication into school. The medication must be labeled and unopened. Do not send in any medication with a child until the school nurse gives the OK. The school nurse is supplied with emergency medications. Please review this general guideline for health services.
