Mandatory In-School COVID-19 Testing

In order for your child to return to in-person learning, you must submit the consent form for in-school COVID-19 testing. Mandatory in-school testing will happen weekly for a random selection of staff and students in all reopened school buildings. If you submitted a consent form before October 15th, please complete a new consent. There is an update in the language of the consent.

If you already have a NYCSA account linked to your student(s): Log in, click your student’s name, click “Manage Account,” and then when a dropdown menu appears, click “Consent Forms.” Read the page, and then choose the consent option at the end for your student. If you need to create an account, please contact Eileen Carrier.

You may also print, complete and sign a paper form. Bring it to your child’s school on the first day they are back. A printable PDF file is available in ten languages at on the NYC DOE COVID Testing page.

Daily Health Screening

All students entering the building must complete a Daily Health Screen. There is paper form that has been updated and can also be shared with your child’s teacher: Daily Health Screen Form.

Travel Out of New York State

All unvaccinated domestic travelers who have not recovered from COVID-19 in the past 3 months are recommended to get tested 3-5 days after arrival in New York, consider non-mandated self-quarantine (7 days if tested on day 3-5, otherwise 10 days), and avoid contact with people at higher risk for severe disease for 14 days, regardless of test result.

Domestic travel is defined as travel lasting 24 hours or longer to states or US territories other than contiguous states (i.e., Pennsylvania, New Jersey, Connecticut, Massachusetts, and Vermont). Travelers must fill out the NYS Department of Health traveler health form
